At Mental Health Defense and Fitness™ PLLC, your care, privacy, and rights are my priority. In accordance with Texas House Bill 4224 and Section 181.105 of the Health and Safety Code, I provide the following information to help you understand your consumer rights.
Requesting Your Health Care Records
You have the right to access your mental health records. To request copies of your records, submit a written request via email to mhdf@mentalhealthdefenseandfitness.com or by mail to 6120 College St. Suite 185, Beaumont, TX 77707.
Include your full name, date of birth, and specify the records you are requesting. Requests are typically processed within 15 business days, and a reasonable administrative fee may apply for copying and processing.
